Responsibilities:

• Collaborate in a multidisciplinary team for bio-psychosocial care.
• Ensure beneficiaries adhere to diabetes protocols.
• Advocate for the rights of type 1 diabetics in the community.
• Build trust, follow up with beneficiaries, and offer social and moral support.
• Work with the Bio-Psychosocial coordinator and Counsellor.
• Assist beneficiaries via phone and in-person services.
• Aid beneficiaries in decision-making regarding education, vision, and goals.
• Liaise and refer beneficiaries to healthcare and social services.
• Engage beneficiaries in beneficial activities.
• Set targets in agreement with beneficiaries and gather relevant information.
• Provide information and support to beneficiaries and their families.
• Offer crisis intervention.
• Maintain accurate records and prepare reports.
• Participate in training, supervision, and meetings.

Social Worker
Requirements:
• Bachelor’s degree in social work or 3 years of experience in the field is required.
• Fully computer literate - Office 365.
• Excellent communication, analytic and listening skills.
• Ability to adapt to all social strata, environment and NGO Dynamics.
• Willing to work during weekends and after office hours.
• Fluent in Creole, French and English.
• Able to develop a treatment plan.
• Knowledge of crisis intervention.
• Respect, empathy and discretion.
• Proactive and self-directed.
• People oriented approach.
• Ability to work in teams.
• Valid driver's license.
